    Information about the person being remembered

    Full Name?
    The person being remembered had a name, but this answer may not be as obvious as it seems!
        Was the person a lady, married? Several times? A format for showing such a relationship is: First Middle-name(s) Last Married name, then (nee) to indicate maiden name and, if applicable, the names in marriage sequence.  Example: Betty Ann Smith married Mr. Jones, Mr. Brown, and Mr. White.  The name response would then be: Betty Ann White (nee) Smith, Jones, Brown. Someone searching for Betty Ann Jones could locate the Remembrance Page.
        Another consideration is title, or rank, associated with a name. Someone who was always referred to as 'Adjutant Michaels', for example, might well be named 'Adjutant Thomas William Michaels' and someone searching for 'Adjutant Michaels' would be able to locate the Remembrance Page.

    There are a number of options available to you in creating the Remembrance Page. Your perusal of these offerings and indication of a selection here is just a suggestion of what may interest you. Actually making payment for one of these options will determine what you wish to place on the Remembrance Page you wish to create.

    Given in the format MM/DD/YYYY, if known. If only the year is known, enter 00/00/yyyy.

    Given in the format MM/DD/YYYY.

    Subject Specifics

    Birthplace?
    Try to include the specific locale: Town, State/Province, Country.

    Where Interred?
    Try to include the specific locale: Town, State/Province, Country.

    Is there an Epitaph?
    If 'Yes', include it in the space provided.

    Brief (500 word) Biography?
    For an example, see Robert Muir Graves.

    Do you wish a picture?
    If 'No', you suggest that you have no desire for, nor possess, any picture at all. And if you have a picture available in digital format (for example .jpg) your response would be 'Yes - attachable'. Perhaps, somewhere, somehow, there is a desired picture already available on the web? Indicate this by sharing the web address (URL). Finally, you may have a picture, but it is under glass or in the family album or in some difficult but possible to reach location. Get assistance in scanning and converting this image to a digital (.jpg) format, or make a copy of the picture and send this to Ever and Forever. Do NOT send something you wish returned! Describe the situation in the space provided, and Ever and Forever will contact you for resolving the picture question.

    Does this person have any references on the web now??
    Perhaps, somewhere, somehow, there is a desired information (in addition to a picture) already available on the web? Indicate this by sharing the web address (URL).

    Name of Charity Link?
    This is totally optional, yet there may be a charity association that you would like to honor. For an example, see Steve McQueen. If the charity you have in mind is not shown, please indicate it by name in the space provided and every effort will be taken to establish this link.

    How is this person related to you?
    Your response helps Ever and Forever place your information in perspective. There may be questions associated with surviving relatives; organizations and affiliations may be a consideration, as well as close friends and business ties.     Submit a request for a Gift Certificate

    Why is the Gift Certificate option by itself?
    If looking for an immediate way to give someone the gift of a Memorial Page, this link will take you there. You will be able to name yourself and provide an email address for yourself, and if available, for your recipient. The information you enter is sufficient to send a copy of the certificate to the recipient, along with an enabling passcode (once your payment has been received).

    Although the Gift Certificate itself is immediately available, the gathering of information may take a few days - perhaps longer, in some circumstances. The balance of this form, including the section 'Gift Certificate Recipient Information', can also be completed now. There will be follow-up communication to ensure that all necessary information has been gathered, but since the subject matter may be emotionally trying for the recipient it is with grace and sensitivity that Ever and Forever would pursue communication with the Recipient of your Gift. Said communication can be, per your suggestion, eMail, voice or regular mail. Whichever seems best for you. No communication with the Recipient will take place until the Recipient makes a separate request, or the Gift Certificate has been purchased.

    What does this cost?
    The prices are nominal, and the amount of information that may be included on each page is considerable. The certificate itself will expose you directly to the currently available plans and prices.

    Of course! Be as explicit as you wish, and indicate your preferred means of contact. Note: voice communications will also require a follow-up synopsis of the conversation by eMail. This ensures your wishes have been documented, and deliverables can be provided consistent with your wishes.

    Another Gift Certificate query?
    If you indicate you wish to give a Gift Certificate, and have not actually done so, Ever and Forever will clarify your intent by responding to this form and answer any questions you may have. You may also complete the information in the next section - 'Gift Certificate Recipient Information'.

    Gift Certificate Recipient Information

    Why fill this section out?
    If you are filling the form out for yourself, ignore this section. If you are sending a Gift Certificate this information includes a regular mail address and a preferred means of contact. Again, No communication with the Recipient will take place until the Recipient makes a separate request, or the Gift Certificate has been purchased.

    Preferred means of Delivery?
    How do you wish the Gift Certificate to reach the Recipient? The Certificate itself will contain a passcode, established only after the certificate has been purchased. By means of verifying the passcode and Name of Recipient the Gift Certificate will be honored.

    What if the Gift Certificate is not redeemed?
    The Gift Certificate is valid for 180 days. At the time of purchase a response indicating the expiration date and passcode will be sent to you as well as the recipient. Only the recipient can activate the Gift Certificate. After the time expires, if the certificate has not been redeemed, no refund will be given. Every effort will be made to ensure follow-up information has been provided and the Remembrance Page created as you intended.
