Ordering Procedures
You will need to consider 2 (two) steps:- Provide the information requested in the form (or Gift Certificate).
- Pay for the service
Upon acknowledgement of the first step by Ever and Forever, payment is made by using the Price Guide and Order Form. Note that Gift Certificates must be redeemed within 180 days, within which time all information must be supplied and the Remembrance Page on this site will be established.

Form for Information Submission
The Form (for input of information regarding the person being remembered) enables you to provide details about someone you believe should be remembered. You will be asked for information about that person, and yourself. Please see our Privacy Statement concerning this information. Upon receipt of this information, Ever and Forever will contact you regarding the details of your request. See Price Guide. See General Content Guidelines.
Payment
Use the Price Guide and
Order Form to place your order for services provided. Gift Certificates are presented to the Recipient in a form acknowledging the Giver and with a password which Ever and Forever will supply. Details regarding the Remembrance Page will be supplied by the Recipient in response to a request for this information (see Step 1).
